Notice
- Only those who had the latest regular health checkups are eligible for health certificates.
- Only on Sep 18, 2026 you can request Health Center to issue this certificates.

健康診断証明書発行終了日のお知らせ（9月入学者用）
2025年度の「健康診断証明書」発行終了日は2026年9月18日（金）です。

注意
- 下記のいずれかに該当する場合は健康診断証明書の発行はできません。
- 大学実施の健康診断を受けていない。
- 健康診断時、未検の検査項目がある。
- 保健室から指示された再検査や校医の診察等を受けていない。
- 自動発行機から発行終了後は、以下の日時まで両キャンパス保健室にて発行可能。
両キャンパス保健室にて2026年9月18日（金）まで。開室時間を確認すること。
